1. Requesting Health Care Records

You have the right to request access to your health care records maintained by our practice. To request records, please submit a written request that includes your full name, date of birth, dates of service, and a description of the records you are requesting. Requests may be submitted through one of the following methods:

All requests will be processed in accordance with Texas law and applicable privacy regulations.
